Medication Reconciliation Summit

Medication Reconciliation is recognized as one
of the most important steps in coordinating our patient’s
healthcare. While writing down a list of medications seems so very
simple, it isn’t until we follow a patient through multiple
transitions of care where medications are changed, added, or deleted
by multiple caregivers that we realize how easily mistakes can be
made. If we add to this outside influences such as therapeutic substitutions
due to drug formulary restrictions, we can easily see why reconciliation
must occur at every change in a patient’s level of care by
trained professionals in varied disciplines.
At the 2007 International MUSE Conference, we met to discuss Medication
Reconciliation. We shared ideas and methods on how we were facing
the challenge of utilizing our MEDITECH and other vendor software
to accurately and completely reconcile medications across the continuum

Joint Commission Implementation Expectations
8.A: (M) C 1. The organization, with the patient’s
involvement, creates a complete list
of the patient’s current medications at admission/entry.

C 2. The medications ordered for the patient while under the care
of the organization
are compared to those on the list and any discrepancies (e.g., omissions,
duplications, potential interactions) are resolved.
8.B: (M) C 1. The patient’s accurate medication
reconciliation list (complete with medications
prescribed by the first provider of service) is communicated to
the next
provider of service, whether it be within or outside the organization.

C 2. The next provider of service checks the medication reconciliation
list again
to make sure it is accurate and in concert with any new medications
to be
ordered/prescribed.

C 3. The complete list of medications is also provided to the patient
on discharge
from the organization.

as you have lots of seasoned peers to assist you!
MUSE is proud to host Part II of the CPOE Summit. Part I at the
2007 International MUSE Conference gave us the “whys”
and fundamentals of “how to”. Part II will continue
where that left off (don’t worry, if you missed it last year
there will be a review).
At the 2008 International MUSE Conference, we will provide you
with solutions to CPOE challenges and guidance in determining which
features to implement. Plus, join us for an extensive Q&A session.
Our panel of participants include users that have years of experience
is the successful implementation of CPOE, so this is a “can’t
miss” opportunity for you!

Technical Summit
Innovative Technical Solutions for
the 21st Century

The 2008 MUSE International conference will be hosting a technical
Summit showcasing the latest technological innovation in Server
Virtualization, Storage Area Networks (SANs), Wireless Networks
and Single Sign On.
Presentations, case studies and discussions will be conducted on
SANs, Asynchronous Replication, Data Snapshots, Disaster Recovery,
and Single Sign On for Access card and Biometric Access to hospital
systems.
This work stream will be led by the Liverpool Women’s NHS
Foundation Trust (based in the UK), who have recently won several
international awards from Information Age, computerworld, and CNET
for their pioneering efforts with these technologies.
At the 2008 International MUSE Conference in Dallas, Texas, you
are invited to join fellow MUSE members for a three-hour sequel.
